Does anyone have an instance where using a link exchange truely helped raise page rank on google?

well I think that it shows some results when you pick up keywords with not such a big number of searches.let's say for "business" or something similar you've got to exchange so many links. Also it helps your PR.If let's say 200 sites and/or blogs link to you, google will like that and give you higher PR then 20 sites and/or blogs link to you.But these sites should be indexed by google.there is two major ways to increase PR with link exchanging:
1.With a lot of links.
2.With not so many links but quality.
3.And there is something like a combination (1 and 2)

How closely 'related' does a site need to be?
For instance if i run a restaurant website do i want to exchange with...
...any travel & entertainment site
...only restaurants
...only restaurants in my area
...only restaurants on my block with similar menus... etc?
It seems the closer you get related the more you are dealing with direct competition & that is counterintuitive.

| One way link is more benificial to increase PR of a site but you can exchange links with other sites which is relevent to your site and have PR higer then your site.Link exchange will increase trafic to your site. |

| I agree, link exchange with sites having higher PR can get you more traffic but the conversions are not guaranteed. For better conversion rate you should get in touch with professional SEO people such as the SEO Services India and get a unique seo plan figured out for your website. |

Yes, link exchange works in boosting pagerank. But it's hard to do. You need to choose your link partners carefully.

There are 100s of examples where the link exchange helps. The only thing you have to keep in mind while doing a link exchange is that, you choose the right exchanger.

Reciprocal links are good but one way links are better than it. if trying for reciprocal always exchange with high PR sites than you.
